WHAT TO EXPECT FROM INITIAL EVALUATION AND HOW LONG ARE APPOINTMENTS?
HOW DO I SCHEDULE AN APPOINTMENT?
Please email us at tara.rudolphi@2empowerpt.com to schedule an appointment. Emails are returned within 1-2 business days. We will provide you with a phone number to talk with your physical therapist directly that will allow your therapist to answer any of your questions.
WHAT ARE YOUR HOURS?
Since we are a concierge mobile clinic and come to you our hours vary and are based on your needs. Each appointment is scheduled directly with your personal physical therapist.
DO I NEED A PRESCRIPTION FROM MY DOCTOR?
As you do not need a physician referral for you or your child to receive physical therapy in the state of Iowa, you may need a physical therapy prescription from your referral source for your insurance to cover physical therapy services. Physical therapy prescriptions or orders can be brought to initial evaluation.
HOW MUCH DOES PHYSICAL THERAPY COST AND DO YOU TAKE INSURANCE?
Our goal is to serve you and your needs at your convenience which is why our business is conducted on a cash-based fee for service basis. Please email
tara.rudolphi@2empowerpt.com for a detailed financial policy and pricing. Payment is expected at the time of service or within 24 hours of service. After each session you will be given or emailed a “super bill” (invoice) for you to submit to your insurance carrier if you choose or keep for your records. The final responsibility for dealing with insurance matters is yours. Under certain circumstances, additional documentation for insurance purposes can be provided upon written request. There may be an additional charge for documentation and reports.

2EmpowerPT does not participate with any networks including Medicaid or HMOs at this time. We suggest that you make contact with your insurance carrier prior to our first visit to determine if physical therapy will be a covered service and to be advised on their procedures for coverage if you will plan to seek reimbursement for services.
As a convenience to you, we accept credit cards. Checks and cash are also accepted. Checks are payable to 2EmpowerPT. You can also pay with HSA account or FSA account. We do not offer refunds on physical therapy services. We are here to assist you or your child in achieving their highest level of success at each session.
GOOD FAITH ESTIMATE
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

Make sure to save a copy or picture of your Good Faith Estimate.
